THE 25 MOST INFLUENTIAL PEOPLE IN BIOPHARMA IN 2015

A lot of people make news in the biopharma business. Few of them are influential.

Influence, simply put, is the ability to make your mark in such a way as to get other prominent people in this business to rethink the way they do business. Last year, that definition caused us to put GlaxoSmithKline's (GSK) Deirdre Connelly on the list for her plan to do away with sales quotas in the U.S. The influential idea then was that ethics had to come before numbers--a big issue at a company that has been challenged time and again by an unethical approach to business that spurred a series of sordid messes and a second take at a painful reorganization.

But influence can have a short shelf life.

Right now, Connelly is no longer employed at GSK, and more than a few analysts say that it's the new sales plan--followed by some dismal numbers on new products--that prompted her departure. Perhaps she should be back on the list this year, for being influential in demonstrating where the real focus was all along on the sales side of GSK.

The people on this year's list of influentials are doing everything from helping change the metrics on R&D performance, crusading new technologies and redefining biotech in a boom, to trying to forge a new definition for Big Pharma--still a hot topic after many of the old definitions failed to generate real growth.

Over the years, we've seen many high-profile individuals adopt a selfless motivation for what drives them. It's all about patients, they'll say. Or honesty. Or putting staffers first. And no doubt sometimes that's true. The truly influential, though, never lose sight of the fact that in order to make a lasting impact on this business, you have to force it to change in some fundamental way. Or get people to look your way while you give it a try.

SANOFI: MAKING A DIFFERENCE WITH 
MOBILE HEALTH
When it comes to pharma's work with mobile health (mHealth) technologies, Sanofi's work within diabetes has established the company as one of the front-runners

Its GoMeals healthy eating iPhone app, released in 2009, was one of the first pharma attempts to harness the then-still-nascent power of Apple's smartphone. Another standout initiative from the company was the iBGStar, an iPhone-compatible device that launched three years ago. The first blood glucose monitor to sync with the iPhone and iPod Touch, it was also the first to allow users to input their own data and notes via a specially designed app.
Continuing this trend, the company last month joined a World Health Organization (WHO)-backed mobile health partnership called 'Be Healthy, Be Mobile' becoming the sole pharmaceutical partner for its diabetes arm.
Pierre Chancel, senior VP of Sanofi's global diabetes division, explained: “Our vision for the development of diabetes is really about making sure that people have better health, better access and better lives.
“This epidemic is skyrocketing and the vast majority of people that desperately need [their diabetes] to be managed or taken in charge are in emerging countries. Two-thirds of people living with diabetes are undiagnosed and the one third of treated patients are out of control.
“The most promising, most pragmatic channel to reach them is, indeed, this mobile health technology”.

mDiabetes

Be Healthy, Be Mobile covers a number of therapy areas and is led by the International Telecommunication Union (ITU) in collaboration with the WHO. It focuses on ways to harness mobile technologies to improve the prevention and treatment of non-communicable diseases like diabetes, cancer, cardiovascular and chronic respiratory diseases, in low- and middle-income countries.

The initial phases of the partnership's mDiabetes programme are taking place in Senegal, where a pilot that ran for the month of Ramadan saw some 80,000 SMS messages sent to approximately 3,500 participants to help patients with diabetes manage their disease. This was followed up by a six-week phase that ran until January 2015 and involved further one-way SMS messaging, with texts send roughly once-a-day.
Senegal is an interesting choice for the pilot. In common with many African countries mobile phones play a particularly important role and in the last year mobile penetration in Senegal reached the 100% mark. “When you look at the number of cell phones, mobile phones that are already existing in these geographies,” Chancel said, adding: “this is the channel to be used in trying to help people living with diabetes.”

A new model of healthcare

Sanofi's senior medical officer and vice president of global medical affairs for its diabetes division Riccardo Perfetti added: “Managing chronic diseases presents problems that are very different from the way healthcare systems worldwide were established for.

“Diabetes is going to be the example, the experiment in public health that is going to prove that that model needs to be integrated with other means to provide three different components, diagnosis, education and management that otherwise would not be accessible just because of the dimension of the disease. 
“One of [our] approaches [is] to be a little bit more pragmatic and understand that the numbers of the disease cannot be addressed by the existing model.”
The mDiabetes initiative is, Perfetti said, “a very tangible and rational way to get closer to patients”. He continued: “So to have devices that may help them to diagnose the disease or tell them what they are doing is sufficient is a critical step”.

Changing behaviour
Patients were recruited to take part in the Be Healthy, Be Mobile's mDiabetes initiative through patient groups or their physician, after which they would be asked to give their consent to receiving the SMS messages.

With Sanofi onboard the next phase of initiative will involve 'two-way' SMS messaging, offering reminders to patients on topics such as their blood glucose levels and food intake.
The 'two-way' part of the initiative will see patients receiving messages such as 'have you thought about monitoring your glucose', and it could also focus on lifestyle habits and eating habits.
“In this area, so many things can be done, but the only thing that counts concrete initiatives, that start, that have a beginning and that have an end,” said Chancel. “So that's where we can get key learnings that can then be developed in order to realise an impact.”
While the pilot and first phases of the initiative were based only on SMS reminders, the second phase - which Chancel told PME is due to commence this month - will be looking to incorporate some mobile app technology.
On Sanofi's plans for this part of the programme Perfetti added: “Clinically, we'll need to be a little bit more ambitious [with the second phase], because if the objective of phase II or step II is really to change behaviour, we really need to collect that [data], so it will need to be a little longer.”
Although this second phase of the initiative will again take place in Senegal, Sanofi's ambitions for this work aren't limited to just one country. “The objective of all of this will be to build something that could be scalable, and this is the whole spirit of the initiative,” Perfetti explained.
“Among the countries we're considering as expansion, Tunisia and the Philippines are some of the countries where this may be a modifier of the way diabetes is managed,” he added.
As to whether Sanofi will bring its mDiabetes initiative to Europe and other large markets eventually, Perfetti said that was also a possibility, with some modifications to account for the region's different technological habits.
“Europe and the US, in terms of familiarity of people with technology, are a little bit more advanced. Perhaps it will need to be tailored to meet the expectation of those countries. But the principle of providing information in an innovative way is still valid”.

10 TEMAS DE SALUD PÚBLICA DESTACADOS EN 2014 EN LatAm

El 2014 ha sido otro año marcado de logros y desafíos para la salud pública en las Américas. Los países de la región han avanzado hacia metas que incluyen el acceso y la cobertura universal de salud, la expansión de la vacunación y el asegurar que menos bebés nazcan con VIH. Pero también han enfrentado nuevos retos como la introducción del virus chikungunya y la posible llegada de casos de ébola.

A continuación, examinamos algunos de los temas de salud pública más destacados sobre los que la Organización Panamericana de la Salud / Organización Mundial de la Salud (OPS / OMS) ha jugado un papel importante junto con sus Estados Miembros, para prevenir enfermedades y salvar vidas.

Avances hacia el acceso y la cobertura de salud para todos

En América Latina y el Caribe, el 30% de la población no tiene acceso a atención de salud por motivos económicos. En septiembre de este año, las máximas autoridades sanitarias de la región acordaron en la OPS/OMS revertir esta situación y trabajar para que el acceso y la cobertura de salud sean universales. "Hemos acordado una hoja de ruta para el avance progresivo para que todas las personas y las comunidades tengan acceso a los servicios de salud que necesitan, cuando los necesitan y sin temor a dificultades financieras", señaló Carissa F. Etienne, Directora de la OPS/OMS. Este año la revista The Lancet publicó una serie especial sobre el tema.

Chikungunya, un nuevo virus que afectó a un millón de personas en las Américas
La fiebre chikungunya, transmitida por mosquitos e importada de África, registró su primer caso autóctono en la región en diciembre de 2013 en una isla del Caribe. En 2014, el virus se diseminó por las Américas infectando a un estimado de más de un millón de personas. La OPS/OMS ha brindado asistencia a los países para que puedan vigilar la enfermedad, identificar y manejar los casos, y preparar y organizar sus servicios de salud. Eliminar los criaderos de mosquitos reduce las posibilidades de transmisión.

Preparando a América Latina y el Caribe para la posible introducción del Ébola

En agosto de 2014, la OMS declaró que el brote de ébola en África occidental constituía una emergencia de salud pública de importancia internacional. Desde entonces, la OPS/OMS ha estado trabajando con los países de la región para asegurar una respuesta rápida ante la posible importación de un caso y evitar la diseminación de la enfermedad. "Evitar la transmisión local es el principal objetivo", afirmó Marcos Espinal, director del Departamento de Enfermedades Transmisibles y Análisis de la Salud de la OPS/OMS.

Bajan las muertes por Dengue en las Américas

La letalidad por dengue en las Américas disminuyó en más de 28% en los últimos tres años, de 0,07% a 0,05%, una reducción que se atribuye al mejor manejo clínico de los pacientes a partir del 2010, cuando se comenzaron a utilizar las nuevas guías de la OMS/OPS. Según estimaciones de la OPS/OMS, unos 1.500 fallecimientos fueron prevenidos el año pasado debido a una mejor atención. Sin embargo, los casos de dengue se quintuplicaron en las Américas entre 2003 y 2013. Este año, la campaña de la OPS/OMS del Día Mundial de la Salud llamó la atención sobre las enfermedades transmitidas por vectores o pequeños insectos, como el dengue, y que ponen en riesgo la salud de la población.

Más tratamiento antirretroviral, menos bebés infectados y nuevas metas 2020

En 2014, un informe de la OPS/OMS arrojó que el número de personas con VIH que reciben la medicación que salva vidas en América Latina y el Caribe se multiplicó casi cuatro veces entre 2003 y 2013. Otro nuevo estudio de la OPS/OMS y Unicef mostró que el número de bebés que nacen con el virus bajó 78% entre 2001 y 2013, resultado de un mayor acceso a los servicios de salud, al testeo, consejería y tratamiento. Durante 2014, los Estados Miembros de la OPS/OMS y socios establecieron nuevas metas para controlar la epidemia, conocidas como "90-90-90". Estas buscan aumentar, para 2020, al 90% la proporción de personas con VIH que conocen su diagnóstico, a incrementar al 90% aquellas bajo tratamiento antirretroviral, y a que el 90% bajo tratamiento tenga carga viral suprimida.

La vacunación y la Copa Mundial de Fútbol

Ante la celebración de la Copa Mundial de Fútbol 2014 en Brasil, la OPS/OMS lanzó su Semana de Vacunación con un llamado a vacunarse contra el sarampión y la rubéola, con el objetivo de evitar la reintroducción de estas enfermedades eliminadas en la región. Este año, la OPS/OMS apoyó también la campaña de vacunación contra el cólera en Haití y la campaña contra el VPH en Brasil, cuya introducción amplió el acceso a esta vacuna a poco más del 80% de las niñas adolescentes de las Américas. En otro hecho relacionado con la vacunación, la Organización distinguió a Ciro de Quadros (1940-2014), pionero de la erradicación de la polio, como Héroe de la Salud Pública.

El suicidio, un problema de salud relevante y prevenible
Cerca  de 65.000 personas  -más de 7 cada hora-  se quitan la vida cada año en las Américas. Este año, la OPS/OMS lanzó su informe Mortalidad por suicidio en las Américas y llamó la atención sobre este problema de salud relevante y prevenible. "Tenemos que detectar los casos de manera temprana, así como también tratar los trastornos mentales como la depresión y el abuso de alcohol", dijo la Directora de la OPS/OMS, Carissa F. Etienne.

Las Américas continua su lucha contra el tabaco

Nicaragua se convirtió en el primer país del mundo en ratificar un protocolo de la OMS que busca eliminar todas las formas de comercio ilícito de productos de tabaco, como la fabricación ilegal y el contrabando. Asimismo, varios países de la región han avanzado en implementar medidas para aumentar los precios y los impuestos sobre el tabaco, lo cual puede animar a los consumidores a dejar de fumar y evitar que otras personas se conviertan en adictos al tabaco. En ese sentido, Panamá y Costa Rica fueron reconocidos con los premios OPS/OMS del Día Mundial sin Tabaco 2014.

El alcohol, responsable de al menos 80.000 muertes al año en las Américas

Un nuevo estudio de la OPS/OMS arrojó que el alcohol contribuye a que al menos 80.000 personas mueran al año en las Américas. En la mayoría de los 16 países analizados, las enfermedades del hígado fueron la causa principal de las muertes vinculadas al alcohol, seguidas de desórdenes neurosiquiátricos. Muchos de estos fallecimientos pueden prevenirse a través de políticas e intervenciones que reduzcan el consumo de alcohol, como restricciones en su disponibilidad, aumento de precios a través de impuestos, y controles en el mercadeo y publicidad.

Bajan las muertes maternas, pero no lo suficiente

La mortalidad materna se redujo en promedio un 40% entre 1990 y 2013 en once países de América Latina y el Caribe, según un informe de Naciones Unidas. Sin embargo, ningún país de la región está en condiciones de alcanzar el Objetivo de Desarrollo del Milenio (ODM) de disminuir un 75% la mortalidad materna para 2015. La OPS/OMS está trabajando para asegurar la disponibilidad de sangre segura para transfusiones durante el parto para que ninguna mujer muera por hemorragia, una de las principales causas de mortalidad materna evitables.
